GP Chronic Condition Management Plans are designed for people with a chronic medical condition that has been, or is likely to be, present for at least six months.

This can include people managing ongoing musculoskeletal, neurological, respiratory or other chronic health conditions, particularly where mobility, strength, balance, pain or physical function are being affected.

Your GP will determine whether a GPCCMP and referral to physiotherapy is appropriate for you.

With an eligible GP referral, you may be able to claim a Medicare rebate towards the cost of your TropiCore Physio home visit. This can make mobile physiotherapy more accessible for people who find getting to a clinic difficult or simply prefer to be assessed and treated in their own environment.

Medicare provides eligible patients with up to five allied health services per calendar year in total, which may be shared between physiotherapy and other eligible allied health professions.

TropiCore Physio accepts GP Chronic Condition Management Plan (GPCCMP) referrals for eligible patients across Cairns. We provide home-based physiotherapy for a broad range of chronic conditions, with experience in neurological rehabilitation, falls and mobility, respiratory and neuromuscular physiotherapy, and complex presentations.

Referring is easy - simply send through the GPCCMP referral and relevant clinical information. We’ll take it from there, contacting the patient directly to arrange their home visit.

Following assessment and treatment, we’ll provide the required communication back to the referring GP and keep you informed of relevant findings or recommendations.
